What are the documents needed for a new BSNL telephone connection?

 The below mentioned are the list of documents you would need to get a new telephone 

connection from BSNL India. Keep these ready with you before you go to get a new connection from them. 

For Identity/Address proof

1)      Passport    

2)      Arms License

3)      Driving License

4)      Photo Identity card having address

5)      Election commission ID card

6)      Any other document containing photograph includes

  a)Ration card with photo &address applicable for person whose photo  is affixed.

  b)CGHS/ECHS Card

  c)Certificate of address having photo,issued by MP/MLA/Group A Gazetted officer in   letter head

                    d)Certificate of address with photo from Govt .recognised educational institutions  

                          for students only

                    e) Certificate of address having photo issued   by Village  Panchayat  head   

                          (for rural areas)

For Identity Proof only

                    a)Income Tax PAN

                    b)photo credit card

For address proof (only)

          1)Ration card

           2)water Bill (not olden than last 3 months)

           3)Telephone bill of fixed line(not older than last 3 months)

           4)Electricity Bill of State company(    not older than last 3 months )              

            5)Income Tax assessment order

Note: For verification of identity/address proof either document listed in Sl No1 or document listed in Sl no.2 &3  (one each for identity& address proof) should be obtained along with Customer Application Form before providing telephone Connections. 

     

Documents to be submitted along with new connections regarding signing authority

1. Photo Identity card issued by Govt.,  or statutory authority

2. Public limited company may enclose certificate of incorporation along with any proof of identity of the authorized officer of the company.

3. In case of Govt., Of India   undertaking , Government of India offices /state Govt .,offices a self certificate on the letter head will suffice along with the name  and designation of the coordinating officer

4. In case of foreign missions in India and other foreign agencies the name and designation of the authorized officer along with details of officials etc., are required for whom the cellular mobile phone is intended,

5. In case of outstation customers details of local references are required.

6. For sole proprietary concern the proprietor may sign himself and affix rubber stamp on the form overhead.

7. In case of partnership  concern , All partners or any one of the partners duly authorized  OR person with  the Power of Attorney may sign. In case of partnership concerns copy of  Power of attorney for authorization 2.Partnership deed in case of company, a copy of the articles of association to be attached.

8. In case of company, signature should be of person on behalf of a company , in accordance with the provisions of articles of association .

9 Public limited company may enclose certificate of incorporation along with any proof of identity of the authorized officer of the company.

FOR EXISTING SUBSCRIBERS

The sub has to fill up the corresponding form .

The authorized signatory should sign.

Originals of the above are to be submitted along with self attested photo copies.  original will be returned after verification.
